Carers Hospital Discharge Coordinator

Barnet Hospital now has a Carers Hospital Discharge Coordinator, Daniel Newland who is available to support unpaid carers when the person they look after is in hospital.

Hospital discharge is often worrying for both carers and patients. Daniel’s role is to help people through this confusing process and continue that support once the patient has been discharged. Daniel can explain what is happening and make sure any concerns you have are discussed with the hospital teams.

The service is very flexible and tailored to individual needs. It is also confidential and impartial, which means that carers can speak to Daniel about all their concerns. The service includes:-

• Interacting with medical teams on the carer’s behalf to ensure that any concerns are considered and that the carer is kept informed of discharge planning.
• Help accessing services such as benefit advice and grants.
• Advocacy, both at the hospital and in the community.
• Informal but researched information about the rights of carers and patients in hospital based situations.

Daniel is employed by Barnet Carers’ Centre, which holds a number of social and informational events and provides counselling for unpaid carers throughout the borough.
